Position Description: Perform dialysis and patient care activities according to company approved policies and procedures
Conduct pre and post dialysis assessments
Perform patient assessments, obtain vital signs and appropriate data, evaluate data and intervene as appropriate during dialysis treatments
Initiate and terminate dialysis treatments on patients with permanent fistulas, grafts or catheters
Discuss changes in patient status with physician and supervisory nurses
Implement patient care plans
Prepare, administer and record medications and intravenous solutions as ordered
Maintain patient home medical records and reports
Provide patient education and reinforcement of education and documents
Participate and direct emergency care as needed to the patient at residence
Evaluate delivery of patient care and communicate to appropriate personnel relevant facts about the patient, action taken and patient responses
Document, verify and ensure implementation of physician orders
Participate in the delivery of patient care
Observe activities and makes recommendations to supervisor for improvement in the quality and effectiveness of patient care
Implement and maintain compliance according to company policies and procedures
Maintain standards of cleanliness and organization of dialysis supplies in assigned patient homes
Perform other duties as assigned
Experience Required: Graduate from an accredited school of nursing
Current licensure as a Licensed Vocational Nurse in the State of Texas having at least 18 months of dialysis experience within the past 24 months
CPR Certification required
Valid drivers license in the state of Texas and proof of valid auto insurance
